i agree with whomever said that you need to talk directly to the vet, and
not assume that your letter (return receipt or not) actually got to her. i
found out the hard way that human medical docs are often incredibly unaware
of what their staffs do and say to patients--and they never find out unless
the patients speak with them specifically.

good vets ARE hard to find, so i'd be wary, too, of tossing one because of
their staff--just refuse to work with that tech, period. you may not be the
first person to have had a problem, but they'll never know if you don't tell
them...
