Yes, they can be used in realtime (on PC, which is all I know). There can be a latency of a few milliseconds, depending on the system. If you use a live soundfont player, the Yamaha Splendid Grand is terrific and free (don't have a working URL at the moment). I can put it on my FTP site at some point, though it's a 250MB download. Also search for the free soundfont Maestro Malmsjo, really fine (and 196MB). There are several Steinway soundfonts which are pretty good, and in a more manageable range (30MB) if you have issues of RAM (most soundfont players don't stream from hard drive).
