Hmm. At least as far as flute goes your statement seems to be quite at variance with my experience. Almost all pros that I deal with use (or at least have available) a flute with a B extension. Stiller gives the percentage as 25%, but at least among pros I think it is much higher now. I routinely write low B for the flute without ossias (and without comment from players).
As for the C extension on the piccolo, I googled it after posting here. (I should have probably googled first, but I'm old enough that it still isn't second nature.)
I found ebay auctions for something called the Schill C Key in various colors. It seems to be an attachment for piccolos that adds the 2 extra notes, but I am not sure that it is something a pro would use. It seems to be marketed at band students. I also can't tell if it is made for a particular model of piccolo, although it seems like it would have to be.
I originally wrote this piece many moons ago when I was young and foolish and mistakenly thought the standard lowest piccolo note was C#. After putting the piece in front of players, I quickly learned my error and revised it. But now in the mellowness of middle age, as I enter the piece into Finale, I realise that I would really like to have those original C#'s if perchance the player has them. So I think I will restore the original notes with the revisions given as ossias.
