There are a few options to open the application:
1) Run the actual executable, which is
/sw/opt/kde4/mac/bin/kwalletmanager.app/Contents/MacOS/kwalletmanager
(kwalletmanager.shell is just a wrapper)
2) Use
open -a /sw/opt/kde4/mac/bin/kwalletmanager.app
since that whole directory constitutes an OS X .app bundle (-a means .app).
3) Navigate to /sw/opt/kde4/mac/bin in Finder and open kwalletmanager
via double-clicking.
kwalletmanager.shell has bogus information encoded into it, which is why
it doesn't run:

It refers to the package build directory, which is removed by default on
a successful build, and therefore is nonexistent.
I'll cc the maintainer about the .shell file. I know he's planning to
update to KDE 4.5, and I wasn't sure if we use the .shell files for
anything or not.
