Here's what I got out of checking mine real quick.

The case should be grounded. The rubber mounts are there to give it a little 
flex while in use, not to electrically isolate it. The little copper strap 
going over to the screw gives it back the ground it loses by being mounted on 
rubber. 

The terminal farthest to the driver's side should get 12 volts with the 
ignition switch on.

The middle terminal should be grounded for high speed.
Both the middle terminal and farthest to the passenger side should be grounded 
for low speed.

Are you sure the motor is spinning in the right direction?  One thing that 
seems like a possiblity is that you have the pos and neg wires connected 
backwards to the motor.  It would work ok when it's not grounded but would turn 
backwards.  When you ground it, it would short out.
