Ok so I have the same advice for you that I already posted for Andrew: try 1.4Xa12 with tracing.
Some of these problems may be due to unusual forms for the URLs. Are you using 'file:' urls? Funky characters? Extra dots or double dots? Some could be due to unusual way of loading source.
