I wonder if you are having the same problem as I'm getting with the Web Developer extension. When I install that it doesn't show up (except in the Add-ons list). Close FF, go to the Profile in Windows Explorer, and delete (preferably rename) the three files called 'extensions.cache', extensions.ini' and 'extensions.rtf'. They will be regenerated when you start Firefox again.
Your profiles are (probably) at 'c:\Documents and Settings\your username\Application Data\Mozilla\Firefox\Profiles\ The problem for me seems to be linked to FF 3.5.2+ and the latest version of the Google toolbar. I also have to say that having Firebug installed MAY be making a difference too (I'm still investigating). The WD toolbar disapears every time I install/update/disable/enable/ uninstall ANY extension, and I have to go through the file deletion procedure as above. Any test of what works or doesn't isn't valid until I've done this. Note also that any extensions you've merely disabled will be re-enabled, so if they are suspect you'll need to uninstall them.
