The PIX OS is called Finesse. (Why it needs a name is beyond me, as it was designed from the ground up exclusively for PIX, and is not used for anything else.)
As for QA, fair enough. But having worked at a number of places prior to Cisco, I'd have to say that the QA process at Cisco is impressively stringent and thorough. You'd be surprised at the number of engineers who test and review each release.
