Testing on 10.4.11 / PPC, I have tried this in 3 configurations now:  
fltk-1.1 (pre-quartz, as it were) fltk-1.3 (pre-cocoa) and fltk-1.3  
(current, cocoa).

Of these, fltk-1.1 and fltk-1.3-pre-cocoa seem to have the sudoku  
menus filled in with the accelerator shortcuts.
The fltk-1.3-current does not have the menu shortcuts filled in.

The sudoku demo does not seem to use the FL_ALT modifier, so I can  
not comment on whether the "saucepan" is upside down or not...

In the 3 cases I have of the menubar demo (which does not utilise the  
SYS_MENU_BAR option and simply uses a conventional fltk menu bar)  
then all 3 examples have the menu accelerator shortcuts filled in.

The fltk-1.1 case the shortcuts are written out in text rather than  
using the symbols, so they look like "Shift+Option+Ctrl+A" and so forth.

On both fltk-1.3 variants they use the "saucepan" and "ctrl ^" and  
"shift-arrow" symbols, there does not seem to be an example using the  
"cloverleaf" symbol. (Side note - there probably ought to be, we  
ought to demonstrate the use of the FL_META modifiers and so forth in  
this demo...)

Anyway, the "ctrl ^" and "shift-arrow" symbols appear to be the  
correct way up, but the "saucepan" alt-modifier symbol appears to be  
inverted.
This seems to be most odd.
