i have had the same problem on my B&W g3/400. in my case, once the problem
starts it is very hard to get rid of. fortunately i had a second partition
with 9.0.4--no problems on that disk. i have very few system add-ons (the
same on both partitions) and run on the original ultra scsi 9gb that shipped
with the machine--and i obviously have usb from the factory.

as this problem is completely debilitating (it only works with *all* exten's
off) i have just stuck with 8.6 on my 350--never once had a problem on that
machine.
