I have this similar card but it only says ADS
USBX-500, doesn't say EF. Mine is only 2 port.  I had
this card working with my Rev A rom Beige G3 233. I
now have a Rev B rom dimm but never had problems with
it when it was Rev A. Make sure it is seated
corrrectly. Make sure all USB drivers are in place.
Reseat it again. Perhaps there's something wrong with
it. Does it work with OS 9? I recall 9.1 worked the
best for this machine. And no problems ever with OS X,
as my Epson 900 only has a USB driver for OS X.
