When you do format it, write zeros to it. That is a true litmus test. I agree the drive is the prime suspect in this...I had a similar problem using copies of the Panther disks. When I tried using the originals it installed without any errors. If you suspect that it's the drive, try to install to another disk and use Carbon Copy Cloner to clone the system to your other disk?
