Is it possible this isn't a Mac card, but rather a PC card? As I recall, it was possible to convert the PC cards into Mac cards, but you had to solder several 10K Ohm resistors over to alternate positions, like jumpers. If you didn't solder these, it was possible to burn-out something on the card. See this:
