Ok, I created another user account and opened Safari from that account,
without doing anything to the existing
Gears installation. Lo and behold - a "Google Geasrs Settings..." item
appears in my Safari app menu. So, good
guess that it was something about my account (rather than the Gears
installation).
I then uninstalled Gears and installed the debug build you provided. I got
all the same behavior as before (no
Gears menu item; no luck taking my Google Docs offline), but there was one
additional message in the Console
log: "PermissionsDialog::Prompt() - Unexpected result." This is generated
when I hit the 'Enable offline access'
button in the dialog I get after selecting the 'Offline' link on my Google
Docs page.
