You should try starting fresh and let GeoServer create the shapefile for
you, the one created by other
software might not be working well.
Also, remove all extra files GeoServer might have created trying to setup
the mosaic (property files, the
sample image, and so on)
If that does not work, we should probably look into what is making up the
mosaic, as there are certain
limitations, for example, all data in the same projection, and same color
model (e.g., you cannot mix
grayscale and color).
