I did try removing the files and starting fresh, with a shapefile generated
with GDALTINDEX. I had the same problems. The images are all the same
projection and colormaps.
This is with GeoServer 2.5.x. I copied the same data to another machine, which
has GeoServer 2.4.x. I found that I was able to create a simple image mosaic
layer with it.
Is it possible something is wrong with the ImageMosaic plugin for 2.5?

Also, remove all extra files GeoServer might have created trying to setup the
mosaic (property files, the
sample image, and so on)
If that does not work, we should probably look into what is making up the
mosaic, as there are certain
limitations, for example, all data in the same projection, and same color model
(e.g., you cannot mix
grayscale and color).
