I have upgraded so I now have a GeoServer 2.6x release. I created a simple
ImageMosaic and this seems to work now.
I probably created the original problem- some of my imagery is in jpeg2000
format as well as Geotif. I was confused on the instructions to add the
jpeg2000 support, and I think I may have copied files to the wrong location. (I
am still trying to resolve the jpeg2000 support, but that is another topic)
Maybe that broke the ImageMosaic?

with GDALTINDEX. I had the same problems. The images are all the same
projection and colormaps.
This is with GeoServer 2.5.x. I copied the same data to another machine, which
has GeoServer 2.4.x. I found that I was able to create a simple image mosaic
layer with it.
Is it possible something is wrong with the ImageMosaic plugin for 2.5?
It is possible, hard to say what without a reproducable test case.
Could you try an official GeoServer release, 2.5.4, and see how it goes there?
2.5.x are maintenance releases, it would also be interesting to see how the
stable series, 2.6.x, works against
your data.
