Here's an update on the 39dollarglasses Transitions authenticity
certificate I was looking for. I contacted on Thursday and Friday
they emailed me back saying it had been mailed October 12, almost 2
weeks after I asked for it. It did arrive with a serial number, but
the LocationID was not on the back of the card. Transitions asks for
it on the form. I tried to register and left it blank and it appeared
to take, so I don't know if my lenses are registered or not. I guess
they'll email me back and let me know. Registration enters me in a
contest to get the price of my glasses refunded, and puts me on a list
for news and special offers.

When my wife ordered her Transitions lenses from SimplyEyeglasses.com,
she got the certificate properly filled out when her order arrived.
Not sure what to think of 39dollarglasses.com over this and the
glasses with crooked temple arms, so the jury is still out if I order
from them again.
