Another possibility which is really easy to do, I have done it, is to hook
up the fuel lines backwards at the pump. I inadvertently did this but
noticed on the ground run that the header tank was not filling. The
pictures in the Coupe service manual are really confusing as to which line
goes to which line on the pump. Because I had done the annual I caught it
and switched lines.Pete

Glad they got through it alive. 

I would no sooner take off with the header tank 1/2 full & 
dropping than I would un-hook my seat belt & step out on the 
wing in flight.  I simply can't understand why a person would 
do that. 

5 will get you 10 that the valve was either closed or that the fuel 
pump top had been left loose at the annual.
