It is literally ALL your messages ... with or without labels or tags or categories or priorities ... except for Trash or Spam or unsent Drafts. If it looks like only 20 messages or conversations are there, perhaps that is because you are seeing only the first page. Look in the upper right of the screen for something like "1-20 of 247" next to the arrows to move forward/backward through the pages. "All Mail" has never failed to show me everything I've got, other than Spam/Trash. *has:nouserlabels* should find every message that has no user label assigned to it, even if it might be part of a Conversation whose other message(s) do have user labels. But if you are trying to locate messages without labels, "*has:nouserlabels*" will indeed do that. One screenful at a time. Personally, I find Gmal's Label system very logical and easy to use.
