Regarding the particular incident, I have already stated, a few times, in this 
mailing list, what happened. Long story short -- it was a mistake and multiple 
failures in processes etc led to the incident happen (instead of catching it). 
The situation was immediately corrected upon discovery, and work is underway to 
improve where failure occurred.

I hope this clarification closes the discussion with regard to the particular 
incident.
