Yes, I agree with your cut.  We have loaded the SNOMED CT mappings for all 
problems and the ICD mappings for all encounter diagnoses. QWe are also using 
SNOMED CT for family and past medical history which we are now testing out.

On the CDM V3 Implementation Call today, there was some discussion on the 
conditions table.  They list ICD9/10/11, snomed ct, other, etc. as types of 
codes.  For the epic sites which have IMO, which maps to multiple standards,  
we questioned how we should handle it.  Load all mappings into the table, or 
just a single preferred terminology. My thought was SNOMEDCT made the most 
sense since it is the Meaningful Use standard.  They are planning to bring the 
question back to the larger group, but I thought you might have some input for 
them as well.
