I was able to use the CSV mapping method Nathan suggested to attach our
i2b2 (HERON) smoking paths as leaves under the relevant PCORI ontology
paths. This worked for `VITAL.SMOKING`, `VITAL.TOBACCO`, and
`VITAL.TOBACCO_TYPE` (Oracle/pcornet_mapping.csv). I also added SQL tests
for each of the three fields (Oracle/cdm_transform_tests.sql).
I'm going to consider this completed unless our EDC results against
production look off, in which case I'll open this ticket back up.
