If patients refuse to accept our advice and we foresee dire consequences, we 
let them sign a document statingthat they understand that they act against  
our advice and have to bear the consequences.

While personally I don't like this paper bullshit, at least it has one 
positive effect: it gets the patients to rethink their decision.

As an IT service provider I would simply state that 
a) yes, if you want it that way, I can do it and match that price
b) no, I am not going to take any responsibility for the outcome because it 
will end up in a disaster with a high degree of likelihood
c) if we do it my way, I take responsibility and guarantee outcome
