A large part of the problem is that all PDFs are not born equal. If you get a message upon loading the file that it is unstructured, good accessibility is problematical.
I can't recall a situation where a link throws me back to the top of the document but, when people don't follow standards, anything is possible. It may help to limit reading to one page at a time. To do that, press ctrl-k in Adobe Reader and arrow to Reading. If you tell it there that a large document consists of one page, documents will be loaded one at a time. Pressing ctrl-pgdn will then take you to the next page. While I quite like that arrangement, some will find it tedious. But you can adjust settings to suit individual situations.
