This will always be true unless Microsoft and Apple developed a built in screen 
reader that will be as good as a third party screen reader. In this case the 
cost of development is spread out to a much large number of sales.

Hi Pat:
I don't think that this is about people wanting a free lunch, it is
the fact that if a sighted person and a blind person wanted to buy
exactly the same pc, the blind person would have to pay more to be
able to make use of it because he/she would have to buy a screen
reader.  That's what upsets some people.
