It isn't free, but I am finding Becky to be pretty good and you can get a demo.

There are a couple of settings that should be changed for best results with a 
screen reader, but it lets you read messages with a cursor without browse mode 
and it handles multiple accounts.  Some of its 
controls are a little unusual but are accessible and the HELP file is a little 
brief.  There is a replacement help file you can get, though.
