I tried to do the survey today, but the questions showed up as text
and not web controls at all. You can't check a box or enter on a
link or choose from a combo box. I tried turning off browse mode and
back on again, but no change. As far as accessibility of this
survey, it fails miserably.

to indicate that you are a Window-Eyes user. I believe you can ignore the
comment at the top of the survey about Window-Eyes users needing to enter
browse mode in order to hear the questions (this to me is showing their
strong anti-GW bias, bringing up an issue that may only apply to older
versions of WE).
