I won't harp on this again... I promise. There seems to be a lot of confusion about this and it can have bad consequences if the wrong assumptions are made.
Thurman, you're right a T1 is a hardwired (maybe) connection between two locations. The network topology that you describe is secure because of the topology not because it is a T1. The T1 is just a time domain digitally multiplexed carrier of 24 64kb/sec DSOs. These DSOs can be agregated for a full 1.54 Mb connection or can be sub-agregated (fractional). They can carry voice and/or data. Let me describe a T1 that is NOT secure. It is the one in my office. I have a T1 connection, the other end of which in located in the offices of a ISP that belongs to some friends. The have an OC3 (45Mb/sec) connection that I get to put my mail server on. This T1 is private in the sense that it carries only what I put on it, but it is NOT secure because it IS part of the public packet switched network we call the Internet. I would not assume that because you have a T1 that it is secure and that firewalls are note required. Not to plug my own profession too much, but it is a good idea to have a network engineer look at your WAN design before you run medical data on it. Thurman's T1, as he describes it, is secure because they are bringing the internet into the network through another channel, and his T1 is already behind a firewall. Best regards, Donald R.
