Not a nut and bolts answer to the question but a practical solution:
The late Pierre Imbert used to emphasize to his students to tune
their instrument while turning the crank at the same speed as the
tune you were intending to play. This eliminates nasty surprises that
can occur when you tune at one speed and play in another. Of course
it gets tricky if you play tunes that change speeds.
I've had this varying pitch problem from time to time, I apply
Pierre's method when it happens. The problem usually goes away by
itself.
