I spent a week or so trading emails with Martin Otten about this - nothing came of it, it's something about pl.v_angle not being sent across to the client (which stops you from getting the correct player eye angles). Quite why this is true, yet all the spectator stuff is done client side is beyond me.
Sorry it's not a fix, but it might point you in the right direction at least - I've not got time to pick it up again right now.
