This seems to be entity related error (model or trigger or something else). Some models are not precached properly and cause map crashes. I have 2 self made TF2 maps that seem to crash upon change from one to another. Errors are:

43/area1_gate1_bottom - func_door: UTIL_SetModel: not precached: *3
48/trigger_multiple: UTIL_SetModel: not precached: *3

    Probably more maps also that I haven't tried. It's very annoying
    to have your full server suddenly empty because of this. Please
    fix it quickly.

            maps use that cause a crash now on the client since the
            servers can actually run the map without a crash. But this
