Ok, more questions. This could just be ignorance on my part, but why are you running ./hlds_i686 as opposed to ./hlds_run? Custom startup and restart script? Are you running any mods? If you are, have you tried starting with absolute vanilla? I would suggest using hlds_run for a vanilla startup, just to eliminate any issues with your start script, if you are using one.
