I have not experienced it on our servers but i have seen it in-game for 
more than 10 times. I'd say 9 out of 10 times, the server has been 
running either Sourcemod, Beetlesmod or HLstatsX. I am not sure about 
that one time, if it was modded or not but i would say that the issue is 
caused by some plugin that can do things related to server maplist cycles.
