Most of those cheaters have only 1 game anyway on those accounts, or its a hijacked one. And if a account is hijacked, it would be bad if all games got vac banned or w/e your gist is there.
VAC bans are since L4D per game if I remember correctly. Before that, it was per engine (HL, source, etc). As for the VAC only being done 2-3 weeks after the offense... I understand valve wants to "bait" the new cheats. But old cheats, that are widely used? should be in a day or so. Reporting on the profile, afaik it IS been looked at. That the kind of report cannot be used due their regulations is something else. I reported once someone with offending (inserted porn) pictures in their steam screenshots etc, was next day that the profile got cleared and the user could not change his profile for a x period. On VAC false positives, I've seen a couple complaints around, and I think one of those was genuine, but as I dont have that data, I cant tell. Its quite strange to see so little complaints about the vac ban system once it has hit. Other game studio's make it a horror, by first not doing anything, then all of the sudden ban a big part of their player base, without even saying that certain stuff was not allowed. See the MW2 config stories etc. I do think valve should improve VAC maybe taking a look at all those anti-cheat plugins out there, and look for ways to put the usable parts of them into VAC. Improving it to such a level that those claims of the makers of their cheat being "VAC Proof" at least cant be true. Its very annoying to see hackers you banned months ago for wallhacking or aimbotting etc still not being VAC'd. Accumulating more and more bans around on servers. Or to see the same hacker back on a different account. I know one case on a community, where a hacker has gone thru near a hundred accounts on their servers. They IP banned 'm, he then uses a different proxy, etc. Afaik, he buys cheap accounts to do this.
