I've never setup instaspawn myself, but according to the valve dev wiki:

To get the norespawntime sv_tag, you need to have set the cvar
mp_disable_respawn_times 1. If it acts like the other sv_tags, I
imagine setting the tag without this cvar will cause it to be
overwritten when the server starts up.
