I had a situation where a piece of plastic broken off of from some part of the printer fell somewhere behind the service station. This caused the wiper mechanism to fail to return completely so the printhead didn't lock. It happened erratically so troubleshooting was rather difficult, and I thought I did see a wrench in the front panel.
Out of desperation I took apart the printer and found the piece of plastic, removed it and the problem went away permanently.
