Is it possible that the volume status is DISALL or NOTCON? But I don't
know why a user with OPERATIONS or UAUDIT could access the datasets.

Is it possible that you have some sort of user exit that does a RACF
check, say using the disk's volser and a class of DASDVOL to make some
sort of determination for access?
