I would guess that the usage goes back to CP-67. The filetype EXEC could 
contain EXEC, EXEC 2 or REXX code, depending on the first line.

I've been hearing "an Exec" (referring to VM) since 1982.
One has to keep in mind, however, that "an Exec" was also used for EXEC
and EXEC2.
