Others have addressed how to get back to OS9 and reset the password.

My concern is with the screen distortion you first experienced. This is
typical of what can happen if OSX is installed before updating the iMacs
firmware. Of more concern is that you have a 350 MHz iMac, these don't have
an external video socket that lets you regain access if the screen display
eventually disappears, as it can.

When you get back to OS 9 go to system profiler and check the firmware is
version 4.19f, if not update it. You will need OS 9.1, 9,2,1 or 9.2.2
installed to do this. Replacing the hard disk won't resolve this potential
problem as the firmware resides in the logic board.
