My idea of setting up a customizable interface with Cold Fusion would be to
just set up the database and when people check their mail with the web
interface it would place their email into it. The CF Interface could be set
up in such away as to assign each email to a folder. Basically, it would
look like folders to the end user, but they would actually be a table in the
database.
The only downside to all this is that if you have a fairly large group of
users the database is going to get large pretty quick if people don't erase
their messages. This would require you to run an Oracle or SQL server
instead of just using an Access database. For our purposes, most of my ISP
users only need to be able to access their mail when they are travelling or
are not at home. This would be perfect for them.

Well, you wouldn't have to use IMAP for that. Cold Fusion could do it all
internal with the use of the database.

I haven't seen the cfx_Imail either but there is an cfx_mail there:
http://www.coldfusion.com/developer/gallery/index.cfm?ObjectID=7440&nocache=
yes
It can do the basic stuff like reading mail, deleting, collecting headers,
attachments, mailbox size.
The one thing missing is to move messages between folders, creating folders,
deleting folders.
I dont know very much about mail (programming) but I guess that one would
have to have an IMAP component to do all the things Imail can do ?

There isn't one; they're just dreaming about it.
