One other thing you might want to check.  We had the same thing with the spam percentage problem.  After checking all our logs we discovered that one of our co-lo's had a php exploit that was pumping out a ton of email.  We went ahead and fixed it for them, sent them a big bill and the problem was solved.

We also got on AOL's list to get feedback reports which showed the emails that were being reported.  All of them came from two different virtuals that we moved to static IPs so that it wouldn't affect our main server IP.  All's well with the world now.
