I found a hole in my system that was allowing relay last week. The messages
were coming through with 10 addresses each. For a total of 14,000 addresses
in 12 hours... I love it when a firewall policy decides not to install.

Point being, if you have rules (we do) spammers or their tools will beat
them in minutes and continue along their merry way.

The death penalty is the best answer to spammers. Kill their connection.

Limit the number of "recipients" per email. I forget if IMail supports it, I
run a couple of email servers..and I'm not able to check at the moment, but
thats what we do. We also limit the number of incoming emails sent to users
on our system..after a certain point (configurable), it slows them down to 1
a minute...or whatever you want. Hardly worth it for spammers. Nothing is
fool-proof. We just try and stay hip to it.
