NAT (I'm assuming its static NAT) is not the problem (that's how my site is running). SMTP doesn't carry needed source and/or destination IP addresses in the application data stream. The best theory I've seen mentioned so far is your firewall has some kind of SMTP proxy running that doesn't know how to handle AUTH.
