Actually, I have worked on systems that DID maintain a 3 9's performance (99.999). It was achieved using Distributed Directors for the front end, redundant connections between the backend systems, and was located in mutiple geographical locations. Any time we would need to update the system we would just down on of the 3 systems, update it, test it and bring it into production as the primary.
