Could this also be a problem with a slow DNS server?  I don't know if there
is any merit to that, but if they are using a external dns that
underperforms, won't that take longer to process the mail?

A slow DNS server may cause the E-mails to be delayed slightly (which would cause the SMTP32.exe processes to stay in memory longer), but the processes should use 0% CPU usage during the time they are waiting for the DNS responses.
