Outlook Express (and Outlook) have a number of known bugs in working with IMAP. Unfortunately, there has never been any market pressure on MS to deal with these, and since they use Exchange in-house, they barely recognize these problems. The problem you described is one of these bugs.
In most cases, the problem can be solved by restarting Outlook Express. If that doesn't work, you can delete the index files for that mailbox at the server. As another test, you can setup the same mailbox on a different machine and test with OE. Finally, OE has a logging option that you can use to see what it thinks is going on. By the way, here are a couple of other common OE bugs: 1. "Invisible messages". Have you ever seen a unread message count (the number in parenthesis after the Inbox) that didn't match the actual number of messages? This occurs when there are messages that OE refuses to open, usually because of something malformed in the header. OE doesn't report an error or anything, unless you use the logging option. It just doesn't show the message. This was a major problem about two or three years ago, and it was common to see many invisible messages. Now it just happens occasionally. 2. Delayed update. In theory, OE is supposed to check for new messages every few minutes (this is configurable) and then update its message list. In practice, sometimes it doesn't do this. I know of many installations where the machine can sit for hours, with OE open, and never tell the user that new messages have arrived. In these cases, the only thing to do is click away from the InBox and then click back. These are the common problems. Less common problems include messages getting stuck in the Outbox, messages not being sent after completing the spell check, and OE losing connection to the server (even though the network connection is fine). Unfortunately, we use IMAP extensively and our clients use OE extensively, so we just have to deal with these things. I just wish MS showed some interest in solving these problems.
