We've found that it's easier to consult with the client on the options of creating a folder or simply modifying the header/subject line. Depending on their preference we set the rules accordingly. Some clients want the folder some want the header/subject mod.

We have a few people who have inbound rules set up, and if those are
triggered, it is sent to a folder called Spam. They can then log in and
check that folder. But they were wondering if there is a way for an e-mail
to be automatically generated telling them they have messages in that
folder.
