I checked with our AFS system administrators here and rule this out.  However,
a colleague offered an excellent guess - these low access counts may come from
unauthorized file/directory access attempts to the volumes.  I did an
experiment
and confirmed this possiblity.  The low access counts may have sustained a long
time because they never get updated anymore after the day of the unauthorized
access attempt(s).

These large number of low-access-count volumes certainly look very suspicious
to be "not being accessed for any good cause at all", and they will probably
keep the storage management cost growing like crazy.
