Do you have the following in /etc/sysconfig/pcmcia ?
PCMCIA=yes
PCIC=i82365
PCIC_OPTS=
CORE_OPTS=
CARDMGR_OPTS=
If you don't have PCMCIA set to "yes", I don't think PCMCIA
services are started at boot time ...
